背景:Patient ownershipは,医師の専門家としてのresponsibilityの重要な要素だが,この概念の理解には重大なギャップがまだある.本研究では,continuity clinicsにおいて,resident,attending,患者の観点からみたpatient ownershipについて研究することで,patient ownershipの理論の作成を探索した.
方法:2019年3月〜8月,constructivist grounded theoryを用いて,2つの小児科continuity clinicのattending, resident, 患者家族に対して27回の半構造化面接を行い,patient ownershipの定義,期待,経験について検証した.継続的な比較分析 (constant comparative analysis)を用いてテーマを構築し,多様な支店を考慮したpatient ownershipを記述する理論を構築した.
結果:Patient ownershipは,患者/家族と医師との間の双方向の関係的なコミットメントとして説明され,感情的・行動的要素の両方を含んでいた.patient ownershipの経験はケアの継続性によって促進され,ロジスティクや他のシステムベースの要因によって,制約されていた.医師は,クリニックスタッフや患者家族を含むメディカルケアチームの一員として見られていた.医師は,スケジュールの制限に基づいて,residentのpatient ownershipをめぐる期待を調整した.
結論:我々のpatient ownershipの理論は,患者/家族は受動的にケアを受けるのではなく,患者と医師の関係に積極的に参加していることが示されている.具体的な期待や仕事は実践の状況次第で変化するだろうが,本研究の結果は,多様な支店からpatient ownershipの見方や研究のあり方を再考させてくれうる.
個人的所感:おそらく外来セッティング寄りであることと,「医師→患者」という方向でなく,「患者→医師」という方向まで含めたpatient ownership…というのが興味深かったです.これは,patient ownershipやpatient care ownership研究としては見慣れない語られ方だったので,また是非精読しておきたい文献.
